Mercy Hospital Jefferson is seeking a compassionate and patient-centered Nurse Practitioner or Physician Assistant to join our established outpatient Palliative Care program. This is an excellent opportunity for an advanced practitioner who is passionate about improving quality of life, guiding patients through goals-of-care discussions, and providing holistic support for individuals with serious illnesses.

Position HighlightsAll patient care is delivered in the outpatient setting, with inpatient involvement limited to communication regarding outpatient referrals.
Responsibilities
Candidates should have a strong level of comfort discussing goals of care and serious illness management. Experience in hospice, palliative care, oncology, geriatrics, or related fields is strongly preferred.
