TLC Nursing Associates, Inc. is seeking a Medical Social Worker for travel assignments in healthcare settings such as hospitals, rehabilitation centers, and long-term care facilities. The Medical Social Worker will assist patients and their families by providing emotional support, resource navigation, and care coordination to enhance their well-being and recovery.

Responsibilities

  • Conduct psychosocial assessments to determine patients' emotional, social, and financial needs.
  • Develop and implement care plans to address patient concerns.
  • Provide counseling and crisis intervention for patients and families.

  • Coordinate discharge planning and transition of care services.

Qualifications

  • Education: Master's degree in Social Work (MSW) .
  • Licensure/Certification:

    • 1+ years of experience in medical or healthcare social work.
